Paper-based Forms

When is a form more than just a form?

Every form has a purpose. However, when a form's final purpose is to ensure the safety and professional care of a person, we know the value placed on that piece of paper.

With over 7 years of experience in designing clinical, financial and human resource forms, we understand the importance of:

  • Accuracy when translating supplied content into a form layout
  • Adherence to corporate branding and style guidelines
  • Binding margins, barcoding and version control for successful interaction with manual and online record keeping systems
  • Human factors (e.g. space allowance for handwritten information, minimum font size, use of colours in a hospital setting/for scanning purposes)
  • Extensive trial and review periods and the need for urgent alterations/fast turn-around times to ensure forms meet hospital/legal requirements

Most importantly, we understand and appreciate the time and hard work behind creating forms for use in large organisations. We offer our knowledge and patience to help you deliver an accurate and expertly developed form.

LiveCycle Smart Forms (eForms)

So why use an electronic form format over paper?

There are a number of important benefits that are behind the increased popularity of such forms:

  • Using a PDF instead of a paper form significantly reduces transaction time, the costs of printing/storage and negetive impacts on the environment
  • A PDF gives the user instant access to the form from anywhere
  • New forms or changes to existing forms can be made available in real-time
  • The dynamic fields allow users to enter more information than a paper form which has restricted space
  • Allows for data set inclusion, validation and auto-calculations, thus reducing the cycle time because the user can’t enter invalid data
  • The submission of complete and accurate data the first time avoids costly and time-consuming corrections
  • Digital signatures may be applied, and the form is submitted electronically
  • Allows for XML data inclusion, which reduces cycle time because data can be automatically extracted and integrated with back-end systems, reducing rekeying of data and resulting in significant reduction of data entry errors
  • When required, these forms can also be designed to cater for print-out/hand completion
